We did a six sigma project which was led by a certified black belt from our organization. The root cause was identified. Various techniques and tools were used ( like Pareto, DDM,Surveys, Interviews etc.) and we came up with a set of process guidelines resulting in substantial reduction in cycle time and increased customer satisfaction.
